http://www.examplemotion.com/legaldoc/view.php/822 Here is judicial council form cm-200 which notifies the court of the settlement. You can also get it off the Judicial Council website. This is filed along with a request for dismissal. The case should be removed from the docket without a motion to dismiss I would think if you file this along with a request for dismissal. Hope that points you in the right direction.
